Birth/Death/Marriage Certificates

Procedure to apply for birth/marriage/death certificate: 

1. Complete application form:

• APPLICATION FORM - MARRIAGE CERTIFICATE Download
• BIRTH CERTIFICATE FORM Download
• DEATH CERTIFICATE APPLICATION FORM Download

2. Send completed application form by postal mail or email to consular@embassyofguyana.be 

3.  If you have a copy of your old certificate, please send by email or post but it is not mandatory. 

4. Make payment:  

     Payment Details - The Embassy only receives payment by bank transfer. Cash is NOT accepted.

     The cost of a Birth Certificate: 3 euros

   The cost of postage by registered mail is 15 euros.

The application is then sent to Guyana for the certificate to be prepared then returned to the Embassy. This process takes at least 4 months.
